Since the beginning of Fairy Tail, Mashima-sensei has highlighted how significant scars are when it comes to Gray. Starting here:
We know he got one in Galuna Island, and one from battling Ultear. And yet, his biggest scar (if it stays a scar that is, but regardless it is a major wound) he self inflicted on himself to protect Juvia:

Even after being “healed”. I don’t recall Mashima-sensei using this pattern with any of Gray’s other scars, especially with them re-opening. It’s important to note that this is a reminder that Gray would do anything for Juvia, including sacrificing his own life. That’s huge. He gets scars from major battles where his need to protect his loved ones is his power. Juvia, being the person he loves romantically, stands out the most.
On a related note: Please don’t be discouraged Gray and Juvia haven’t properly spoken yet. When Gray said he would give his answer “after the war” and to “let him focus” really may have been Mashima-sensei saying that to us as well. Our canon moment will come in time, because frankly there is no where else left for them to go.
